Please remember that 30th April 2013 is the deadline for submission of the following CS&PF forms:
- Solo and Relay Swim applications
- Medical Assessments
- Payments of swim and administration fees
Your properly completed and signed Relay or Solo Application and Medical forms must be received by CS&PF in the same envelope by no later than 30th April, failing which your application will be invalid.
